Hazel and Augustus went to lunch at Restaurant LA Rive which serves modern French and Mediterranean food.  After being seated by the HOST Hazel and Augustus IMMEDIATELY placed their NAPKIN in their laps. While sitting in the restaurant they were able to see the BUTCHER preparing the meat and poultry. He then sent the meat over to the GRILL chef who grilled them. The WAITRESS came over to take their order.  Hazel ordered some sautéed vegetables, they were prepared by the SAUTE chef.  Augustus decided he wanted salmon in a butter sauce, so the waitress told the FISH   chef. The restaurant was extremely crowded so the RUNNER   helped the waitress bring the meal to the table. After the main course they decided to order desert, because it was a special occasion the PASTRY   chef brought out a chocolate mousse and mini tarts for the couple.

Hazel remembered that the plate was placed in the CENTER of the place setting.  The knife was to be placed to the RIGHT of the plate and the spoon was NEXT  to the knife. Hazel forgot to turn the blade of the knife TOWARDS  the plate.  Augustus came by for dinner and he helped Hazel finish setting the table. He placed the fork to the LEFT of the plate. The napkin was placed UNDER  the fork.  Augustus then placed the water glass ABOVE  the KNIFE.
